Yesterday my partner and I had a prescreening visit + visit 1 on a clinical trial. This was the longest visit for the study. Dr. Newberg & Jackie’s study team kept the processes moving and the atmosphere was cheerful and friendly. It felt good to work together with them to further vaccine research. We’re looking forward to our next visit when Christkindl market will be open since it’s only a block away. Parking validation is provided. Thank you everyone. 😁

Very unprofessional, unorganized and poor communication on behalf of Patient Service Manager, Antoinette Velez. I was not told where to park nor did I receive email regarding where to park. I parked in the parking garage across from them. It was not the right spot. Instead it was supposed to be at 181 N. Dearborn across from Goodman Theater. I had to go down and move my car and park at 181 N. Dearborn. I come back and waited 1/2 hour for the Reception to come back from the bathroom. The lady Anna move me around too many times and not getting to the point from the beginning. Fill out questionnaire and met with the Medical person. Waited another 1/2 hour saying that I am not qualified for the survey. Then I receive my payment card for the survey. I was 1/2 way to the Parking Garage. Antoinette had the Reception to call me to say I need to fill out W-9 Form for the Survey. I said is that really necessary since I did not get qualified for the survey. The W-9 is for receive over $600. At this one-time payment is way less than $600. She said that if I do not come up that they will not load the payment on my card. Basically, Antoinette threatened me. I was very aggravated because the traffic was getting heavy and construction on expressway. When I came back up for 3rd time, she is arguing with me and saying that she sent me email about the parking. I said, "You did not." Pretty much she doubted me. More and likely I will not get qualified for a survey a long time and it is certainly not going to be this year with 4 1/2 months left of the year. I am arguing with her that is ridiculous with the W-9 form with a small amount, it is not over $600. VERY UNPROFESSIONAL, UNORGANIZED, AND POOR SERVICE I EXPERIENCED.
